Changes to NYS Health examination requirements

Dear Parent/Guardian and Health Care Provider,

This is to inform you of two important changes in Education Law effective 7/1/2018.

Beginning 7/1/18 all New York State public school students must have a health exam when they enter school as a new entrant and in Pre-K or Kindergarten and grades 1, 3, 5, 7, 9, and 11. These examinations must be completed on the approved NYSED Student Health Examination Form for School for the health exam.

We understand that your medical provider’s office may not yet be aware of the change, so if you have already had a physical examination completed for the 2018-19 school year on a different form, the school may accept the physical on that form.

The Approved NYSED Student Health Examination Form for Schools will be available this spring on the BKW school website.

In 2019-2020 ONLY the approved form will be accepted.  We realize many offices use electronic health records to document student health data. We encourage you to work with your EHR vendor to incorporate the approved form prior to 2019.

We appreciate your time in collaborating with us to provide your child/patient and our student with the required documentation as required by law.
